Supported Living-Treasure Coast Community Caring
For individuals that choose to live in their family home, a shared home, or their own home; residential services can provide needed supports tailored to the individual's needs:
Supported Living (Capped at 20 hours monthly)
Supported Living Coaches serve many functions in case management of medical, legal, and financial matters..finding a home, paying the bills, renewing a lease, going to the doctor, responding to emergencies, preparing shopping list, etc. They are also tasked with preparing and training the individuals on their Implementation plan. This service coordinates with the other service providers to ensure care plan is followed.
Personal Supports (In-home) (Can be 10 hours week or 24 hours around the clock)
Personal Support Workers assist with whatever is necessary for the day such as: showering, administering medication, cooking, helping clean home, grocery shopping, social skills, etc. May have some overlap with other services such as SL Coach, since coach has limited hours.
Respite (Usually limited hours )
This service can be provided in individual’s home or group home if available. This is usually for short term relief for primary care provider and mainly involves keeping individual safe and healthy, with little specialized therapy.
Personal Care (Very limited hours daily)
For additional care for families who are the main support provider or when other supports such as Personal Supports is not sufficient . For assistance with bathing, grooming, dressing, meal preparation and eating.
Companion (Life Skills development 1)
This service is usually for assisting individuals access the community for socialization. It is specifically for non medical activities and can be utilized 1:1 or up to 1:3 ratio.. This service is often used in lieu of ADT for a “meaningful daytime activity”, it is an option for residents of own home or group home.
